Summary of content The focus of this module will be on promoting heart failure care management that is proactive and organised around the concepts of needs assessment, planning and prevention. Learning outcomes arrow_forward On successful completion of this module students should be able to: 1. Synthesise the evidence base relating to the determinants that link socio-economic variables to the development and progression of heart failure. 2. Explain the physiological and pathophysiological intricacies associated with sequential development of heart failure and apply in depth knowledge of abnormal physiology to analyse and interpret information associated with individual patient characteristics. 3. Undertake a comprehensive assessment (including history-taking and clinical examination), encompassing physical, psychological, social, occupational, emotional, spiritual and caregiver assessment; and use the collective outcomes to inform diagnostic and therapeutic interventions associated with evidence-based prescribing, care planning, implementation and evaluation of appropriate person-centred care and support from diagnosis through to end of life. 4. Critically appraise the role and collaborative contribution of the wider professional team working across primary, secondary and tertiary health and social care settings for optimising care outcomes for persons living with heart failure and their carers. 5. Critically evaluate the impact of comorbidities on the overall management of chronic heart failure. 6. Map and appraise personal and professional progression, aligned to national and local competency frameworks, to evidence ongoing professional development within their heart failure role.
Teaching / learning strategy arrow_forward The teaching and learning strategy within this module will actively guide students to appraise theoretical content and apply this to their professional and personal experiences. This will be achieved by engaging in academic debate, discussion, case analysis and simulation to ensure that shared inter-professional experiences may be fully utilised in the learning process. This approach will encourage the development of critical thinking and evaluation to enable the student to self-reflect, recognise and acknowledge the wider impact of advancing illness within any professional context. Relevant clinical practice skills and knowledge are developed and consolidated through supervised practice in the student's workplace, supported and assessed by appropriate clinicians with expertise in the management of chronic heart failure. Students are required to achieve the minimum pass mark in all assessed components to successfully pass the module. Blended delivery Blended delivery will encompass a mix of both face to face and online delivery and will encompass a range of teaching and learning strategies including lectures, seminar activities and group work to promote creative, interprofessional. self-directed learning both in the classroom and using the virtual learning environment, GCU Learn. All students All students will be required to submit a formative assignment in preparation for their summative submission.
