Summary of content

The aim of this theory-based module is to develop knowledge, skills and understanding that will enable students to take a critical approach to managing and leading a range of care providers in the delivery of compassionate person and family centred care. The focus will encompass the trajectory of illness from onset to end of life and include the management of the clinically deteriorating patient.
The module will nurture students' ability to create and support a quality culture, managing risk, and facilitating efficient and effective use of resources. In addition effective coordination and communication between individuals, families and health and social care professionals will be promoted to provide integrated care to support individuals in meeting their personal health goals.
Students will critically evaluate the implementation and embedding of anticipatory care in proactive approaches to care planning including self-management, end of life care and advanced care planning. Strategies to empower and enable individuals to make informed choices about their own health and wellbeing and discuss any concerns with those close to them or involved in their care will be appraised.
