How to order SNDRí diet-sheets

SNDRí’s annual pricing review - new prices take effect from 1st April 2010. 
  • Orders using 2009/2010 prices must be received by Friday 19th March 2010
  • All orders received on or after 19th March 2010 will be subject to new prices.

Always refer to the most up-to-date order forms for correct prices and ordering information.

1. Download an up-to-date order form.

This order form is password protected; registered dietitians should request a username and password by contacting the SNDRí team.

This order form contains all diet-sheets from the HP and PiL ranges.

2. Complete the order form by filling in all parts including the:

  • customer information
  • delivery details, costs* and payment information.

    *The following prices apply for postage up to 30kg:
    - Scotland £5.60
    - Highlands £22.57click here for a full list of applicable towns.
    - Scottish Islands £28.60 click here for a full list of applicable islands.
    - England & Wales (mainland) £6.60
    - Northern Ireland and Ireland £30.07

  • Incomplete forms will not be processed.

3. Send your completed order, together with the payment method (purchase order, cheque or credit card details) to McCormicks:

4. Please have all reference codes relating to your order available when querying your order.

Please note:

These items are VAT exempt; do NOT add VAT to your remittance.
An official NHS order form must accompany all orders from NHS Trust, Health Boards and Health Authorities.

  • Do NOT send payment with NHS orders; John McCormick & Co. Ltd. will invoice your Finance Office directly.
  • Payment should be made in Pounds (£) Sterling upon receipt of invoice.

For orders from out with NHS, please send a cheque made payable to John McCormick & Co. Ltd. with the completed order form.
